5. Copy of Business Registrations - for self-employed applicants
6. Copy of Latest Income Tax Return (ITR) with BIR Stamp; For Self-employed ITR must include pages with Net income and total revenue
7. Original Bank Certification - for 6 months
8. Original Bank Statements - for 6 months

11. Daily Itinerary
12. PSA Birth Certificate and PSA Marriage Certificate -  issued within one year; if travelling with Family
13. For Sponsored Trip: All documents plus sponsors documents including Invitation/endorsement letter
